Remission Period after Phototherapy in Psoriasis Patients: A Nationwide Population-based Study in Korea

Abstract
Background: Narrowband ultraviolet B (NBUVB) phototherapy is a frequently used and effective systemic treatment for psoriasis. However, recurrence after successful end of phototherapy in psoriasis patients is often observed in clinical situation. Big data studies on factors related to the remission period after NBUVB phototherapy is lacking.
Objective: This study aims to figure out factors associated to the remission period after NBUVB phototherapy in patients with psoriasis.

Methods: This study is based on the nationwide insurance claims data from 2014 to 2018 provided by the Health Insurance

Review & Assessment Service. Patients diagnosed as psoriasis and received phototherapy for psoriasis without concurrent systemic treatment were included. Period from the end of phototherapy to the restart of any systemic therapy was defined as remission period. A series of phototherapy performed more than 5 times without more than 30 days of interruption was defined as a cycle.

Results: Total 29,085 psoriasis patients (15,262 male/13,823 female, mean age: 45.91¡¾15.53 yrs) were included. Remission period was significantly shorter in patients with history of systemic treatment before 90 days from the cycle of phototherapy (p<0.001) and in patients whose treatment duration of the cycle of phototherapy is longer than 90 days ( p< 0.001). Age less than 60 years and treatment interval within a cycle longer than 5 days were significantly associated with short remission period in primary care clinics ( p= 0.001 and p< 0.001, respectively), but not in secondary or tertiary hospitals.

Conclusion: This study suggested that low disease activity and dense treatment within a cycle is possibly related to the longer remission period of phototherapy in the patients with psoriasis.
